Is this generator HIPAA-certified?
No — there is no formal "HIPAA certification" for fax cover sheet generators or templates. HHS does not certify or endorse any specific tool. This generator builds a customized fax cover sheet that includes a CONFIDENTIAL · CONTAINS PROTECTED HEALTH INFORMATION (PHI) banner and a pre-printed PHI confidentiality notice — content commonly used in medical contexts. Whether your overall fax workflow meets HIPAA depends on your organization's administrative, physical, and technical safeguards as a whole.
Is Apollo's Templates a HIPAA Business Associate?
No. We provide a downloadable template builder as a tool. The generator runs entirely in your browser — your logo, company name, and contact details are processed locally and never reach Apollo's Templates servers. We don't access, store, transmit, or process Protected Health Information on your behalf, and we don't enter into Business Associate Agreements.
Where does my practice's logo go when I upload it?
Into your browser, then into the PDF on your machine, and then to your hard drive when you click Download. It does not pass through Apollo's Templates' servers. The generator is JavaScript that runs on your computer; the upload step uses your browser's local FileReader API.
What does the confidentiality notice say?
It tells unintended recipients that the fax may contain confidential or privileged information protected by federal and state law (including HIPAA where applicable), that they must not read, copy, distribute, or disclose any part of it, and that they should notify the sender immediately by phone and destroy the transmission. The wording is generic boilerplate adapted from widely published healthcare confidentiality notices — have your compliance officer or legal counsel review it before adopting it for your practice.
Can I customize the confidentiality notice wording?
Yes — the generator includes a "Confidentiality notice (editable)" textarea pre-populated with a generic boilerplate. Replace it with the exact wording your compliance officer or legal counsel has approved for your practice — for example, additional phrasing required by state confidentiality statutes (California CMIA, Texas HB 300, New York PHL §18). Whatever you type prints directly inside the bordered notice block on the PDF and updates in the live preview as you type. Click "Reset to default" if you want to revert to the boilerplate.
Should I include patient name on the cover sheet?
Best practice under HIPAA's minimum-necessary standard is to use the least-identifying information that still gets the fax to the right recipient — patient initials or an MRN in the RE / Subject line is more common than a full patient name. The generator produces a clean cover sheet with the standard fields; you fill them in per fax following your practice's own minimum-necessary policy.
What about misdirected faxes?
Misdirected PHI faxes are a common HIPAA breach scenario. Best practice: call the unintended recipient immediately and ask them to destroy the fax (the confidentiality notice on this cover sheet primes them to do this), document what happened, and follow your organization's breach-notification procedure. Whether the incident requires a notification under the HIPAA Breach Notification Rule depends on a four-factor risk assessment — consult your compliance officer.
Why a different page from the main /fax/generator/?
We intentionally keep the HIPAA generator on its own URL so the prominent disclaimer language and the CONFIDENTIAL banner are tied to the healthcare use case. The underlying engine and the three designs (Atrium, Forum, Beacon) are the same as the standard generator — only the HIPAA top banner and the pre-printed PHI confidentiality notice block are added.
