About this format

Why the FACSIMILE wording.

The formal version of FAX — legal, government, and financial-services correspondence.

“Facsimile” is the formal, full-name version of “fax” — most commonly seen on legal documents, government cover pages, and old-school formal business correspondence. Choosing FACSIMILE over the everyday FAX wording is purely cosmetic, but in some contexts (a court filing, a federal-agency cover, a financial-services compliance fax) the formal terminology reads more in keeping with the documents it’s wrapping.

The mechanics are identical to the FAX version: click into each named field (Attention, From, Send Fax, Return Fax, Date, Pages, RE, CC), type your details, tick the delivery checkbox, and print. Every PDF is AcroForm-fillable in any modern browser or PDF reader, on every operating system. No signup, no email, no watermark.

If you’re sending protected health information (PHI), add your firm’s standard HIPAA confidentiality notice to the Comments block — these templates aren’t pre-stamped with one yet, but the field is generous enough to fit a full paragraph notice. For attorney-client privileged content, the same approach works for a privilege notice.

All six designs (Onyx, Sienna, Sage, Indigo, Marigold, and Slate) ship in both wordings — pick the FAX version if your office uses casual language, or this FACSIMILE version when formality matters.

Frequently asked questions

When should I use the FACSIMILE wording instead of FAX?
Choose FACSIMILE for court filings, government-agency cover pages, and formal financial-services correspondence, where the full formal term reads in keeping with the documents it wraps. It's a purely cosmetic choice — the fields, layout, and fillable behavior are identical to the everyday FAX version.
Is a FACSIMILE cover sheet accepted for court or government filings?
A cover sheet is just the routing page that rides on top of your transmission — acceptance rules are set by the receiving court or agency, not by the cover sheet. Check the destination's filing instructions for any required language; the FACSIMILE wording here simply matches the register of formal filings.
Can I add an attorney-client privilege or confidentiality notice?
These PDFs aren't pre-stamped with one, but the Comments field is generous enough for a full-paragraph privilege or confidentiality notice — type or paste yours in before printing. If you're sending protected health information, use the HIPAA collection at /fax/hipaa/, which ships with a pre-printed PHI notice.
Are the FACSIMILE and FAX versions otherwise identical?
Yes. All six designs (Onyx, Sienna, Sage, Indigo, Marigold, Slate) ship in both wordings with the same fillable fields and the same layout — only the title word on the page differs. Pick whichever matches your office's tone.
