Resignation Letter for Health Reasons
You don't owe your employer a diagnosis. The letter inside these templates resigns with a date, says 'for health reasons' and that you can't share details, thanks the team, and includes the one clause a medical resignation needs that others don't: if your condition requires leaving sooner than planned, you'll say so at once and work with HR. Printed in full below; free in Word and Google Docs, in a traditional sage, a traditional navy, and a minimalist page.

[Month Day, Year]
[Manager Name]
[Title]
[Company Name]
[Company Address]
Dear [Manager Name],
Please accept this letter as formal notice of my resignation from my position as [Job Title] at [Company Name], effective [Last Day — two weeks from today, or an earlier date if your health requires it]. I am resigning for health reasons that make it impossible for me to continue in the role at this time.
I am not able to share the details, and I appreciate your understanding of that. This is not a decision I have made lightly; my time at [Company Name] has meant a great deal to me, and I am grateful for the support I have received here.
I will do everything I can to make the transition smooth before my last day — [documenting my work, handing off open items, briefing a colleague]. If my condition requires me to leave sooner than planned, I will let you know immediately and work with HR on the details.
Thank you for the opportunities and the kindness you have shown me. I wish you and the team continued success.
Sincerely,
Eleanor Whitfield
What a health-reasons resignation letter should say.
A medical resignation is read by a manager and, usually, by HR. Both need the same four things and neither needs the diagnosis:
- The resignation and the date. ’…effective [date]’ — with an earlier date if your health requires it. State what you can commit to.
- The reason, in a phrase. ’For health reasons that make it impossible for me to continue in the role at this time.’ That is the whole explanation; ’I’m not able to share the details’ closes it.
- The earlier-exit clause. ’If my condition requires me to leave sooner, I will let you know immediately and work with HR.’ It protects you and sets expectations.
- The handoff you can manage. Documentation, open items, a colleague briefed — scaled to what you can actually do.
Before you resign, ask HR about medical leave — FMLA in the U.S. protects up to 12 weeks for eligible employees, and short-term disability may apply; resignation is sometimes the second-best option. If today is the day, the immediate resignation letter has health and illness versions. If the health issue is stress from the job itself, the stress and burnout letter says that without details either.
Fill it in and send.
- Ask HR about leave before you resign — FMLA or disability may fit better than quitting.
- Download a template; fill the brackets: last day (with the earlier-exit clause kept), what you can hand off.
- Export a PDF, tell your manager first, keep a copy for any later benefits claim.
